Licensed Practical Nurse (LPN) in Long-Term Care (LTC)

The LPN must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served. The individual must demonstrate knowledge of the principles of growth and development across the lifespan. The LPN must possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed. The LPN is responsible for providing direct and indirect skilled nursing care for assigned residents.

The LPN follows defined policies and procedures under the supervision of a Registered Nurse. Maintaining collaborative relationships with other nursing staff, physicians, and ancillary staff is essential. The LPN must maintain an appropriate level of knowledge and clinical expertise through continuing education and in-service programs. The LPN should have the ability to provide care to geriatric residents.

The LPN collects data relevant to the resident's physical and psychosocial condition on an ongoing basis. The LPN communicates findings to the Registered Nurse and other members of the Interdisciplinary Team. The LPN contributes to the creation of a developmentally age-appropriate and culturally relevant plan of care. The plan of care should reflect nursing diagnoses and interventions to achieve desired resident outcomes.

Involving the resident/family and healthcare team in the care process promotes a healthy and safe environment. The LPN recommends revisions to the plan of care after evaluating resident outcomes. The LPN indicates the need for referrals to ensure all aspects of the resident's care have been addressed. Collaborating with the resident/family and members of the Interdisciplinary Team is crucial for customer satisfaction.

The LPN maintains current knowledge and competency in Nursing Practice. Participating in the mentoring or orientation of staff is beneficial. Safety, cost, and impact factors should be considered in planning care delivery. Under the direction of the Registered Nurse, the LPN provides leadership in the professional practice setting. Demonstrating knowledge and competency is essential in this role. Occupational exposure to blood borne pathogens is reasonably anticipated.
