Description:

Under the direction of licensed nursing personnel, performs direct and indirect designated resident care within the scope of practice as defined by the certification. The LPN must demonstrate the knowledge and skills necessary to provide care appropriate to the age of patients served on the assigned unit. The individual must demonstrate knowledge of the principles of growth and development across the lifespan.

They must possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to their age-specific needs. Resident's activities of daily living (ADLs) needs are met as outlined in the plan of care and in accordance with policies and procedures within a safe and comfortable environment. This includes hygiene, grooming, dressing, incontinence/catheter care, elimination/toileting needs, turning/positioning, and skin care.

Documentation is completed per protocol. Resident's environment and safety are maintained as outlined in the plan of care and in accordance with policies and procedures. This includes answering call lights and alarms promptly, provision of safety mats and low beds, and maintaining cleanliness of the unit. Restraint use is under the direction of nursing, and appropriate use of and monitoring for malfunctioning equipment is practiced.

Documentation is completed per protocol. Resident's nutritional needs are met as outlined in the plan of care and in accordance with policies and procedures. This includes meal delivery, tray preparation, adaptive equipment, and feeding. Resident's preferences and choices shall be honored. Documentation is completed per protocol. Resident's rehabilitative (restorative and maintenance) needs are met as outlined in the plan of care and in accordance with policies and procedures.

This includes range of motion, ambulation, transfers, proper body alignment, and use of assistive devices. Documentation is completed per protocol. Resident's rights, refusals, and concerns are reported to the Charge Nurse and/or Supervising Nurse in a timely fashion in accordance with policies and procedures. Changes in resident physical, emotional, and psychosocial status are identified and reported to the Charge Nurse promptly.

Occupational exposure to blood borne pathogens is reasonably anticipated. Required tasks involve potential contact with blood, body fluids, or tissues. The LPN must exhibit professionalism and compassion in all interactions with residents and staff.
