MDS Coordinator / Registered Nurse Assessment Coordinator (RNAC)

Riverfront Rehab is seeking a detail-oriented and experienced MDS Coordinator (RNAC) to join our administrative team in Newark, NJ. The MDS Coordinator is responsible for determining resident acuity levels, managing the Resident Assessment Instrument (RAI) process, and ensuring accurate documentation to maintain regulatory compliance and optimize reimbursement.
Key Responsibilities- Assessment & Care Planning:
Coordinate the interdisciplinary team to complete MDS assessments and Care Plans within Medicare/Medicaid schedules. - Revenue Optimization:
Track Medicare eligibility and predict RUGs categories; perform concurrent reviews to ensure clinical information accurately reflects the care provided. - Compliance & Data Management:
Oversee the electronic transmission of MDS data to state and federal agencies, ensuring accuracy, timeliness, and strict adherence to legal and safety codes. - Collaboration:
Lead daily admission and care planning meetings; communicate acuity changes to the Medical Director, Administrator, and Director of Nursing. - Quality Control:
Conduct regular audits of medical records to validate that documentation supports MDS coding and review final validation error reports. - Staff
Education:
Train facility staff on MDS terminology, documentation requirements, and reimbursement regulations to enhance overall clinical data integrity.
- Current Registered Nurse (RN) license in New Jersey (Required).
- 2+ years of clinical nursing experience in a long-term care (LTC) setting.
- Proven experience with MDS 3.0 completion, RAI processes, and Medicare/Medicaid reimbursement.
- Education:
Graduate of an accredited school of nursing; BSN preferred. - Skills:
Strong organizational abilities, deadline-oriented mindset, and thorough knowledge of LTC regulations.
