Job Summary
Under the direction of the Manager, the Hospital at Home Supervisor of Care Coordinators will supervise the screening and identification of qualifying patients utilizing defined social and clinical eligibility criteria for the Hospital at Home program. The Supervisor of Care Coordinators will oversee the coordination of all communication related to completion of the physician clinical screening for potentially eligible hospital at home patients and provide leadership for the care coordinators for the orchestration of admission/transfer activities for the hospital at home program.
The supervisor is a leader in the provision of quality care to patients through the implementation of the nursing process applied through a team nursing model while maintaining high quality and safety standards.
- Supervise the assessment, identification, and documentation of qualifying patients for the Hospital at Home program through the use of pre-defined social and clinical eligibility criteria.
- Oversee care coordinator collaboration with a team of multi-disciplinary clinicians to facilitate the patient admission process into the program and communicate required patient logistics required for admission and ongoing clinical care.
- Set and drive achievement of daily goals for Care Coordinators for achievement of operational results.
- Supervise the distribution and education of patient and family on the use of the telehealth tablet used for biometric data capture, patient symptom reporting, and telehealth visits, as well as any other outlined equipment and supplies needed for patient care.
- Ensure performance standards are achieved, including patient and family education.
- Supervise Care Coordinator colleagues ensuring adherence to policies and procedures of the organization, including Human Resources, HIPAA and patient confidentiality regulations, and ensure policies, practices, and procedures are understood and followed by Care Coordinators.
- Understand regulatory directives and maintain compliance with all regulatory guidelines.
- Complete departmental reports within designed time frames.
- Ensure coverage and that colleagues are appropriately trained.
- All other duties as assigned.
- Minimum of 3 years of acute care nursing experience required.
- Experience working in an emergency department setting preferred.
- Ability to function independently and understand complex care management.
- Graduate of an accredited nursing program.
- BSN preferred.
- Current State of New Jersey nursing license to practice as a registered nurse.
- Current CPR certification and BLS.
- Valid Driver's License required.
