Title:

Grants Manager Department: WMCHC Administration Mission:
The Grants Manager identifies, develops, and manages grant funding opportunities that support Wayne Memorial Community Health Centers’ mission and strategic priorities. The role is responsible for the full lifecycle of grants, including research, application development, award management, compliance, and reporting; ensuring all grant activities align with federal, state, and private funding requirements while supporting operational and financial goals. The Grants Manager works collectively across departments to develop compelling proposals, track outcomes, and ensure successful execution of grant-funded programs.

The role also supports federal program requirements, including FQHC- related deliverables and reporting.

Responsible to the Chief Development Officer (CDO)
Duties/Responsibilities:

Grants Strategy and Development Research and identify local, state, federal and private grant opportunities aligned with organizational priorities

Develop and write grant applications, proposals, and supporting documentation

Collaborate with internal stakeholders to gather program data and information, outcomes for grant submissions

Ensure all applications are complete, accurate, and submitted within required timelines

Grant Management and Compliance Manage the full lifecycle of grants, including pre-award, award and post-award activities

Monitor grant requirements to ensure compliance with all funding conditions and regulations

Track grant deliverables, timelines, and reporting requirements

Prepare and submit required progress, financial, and compliance reports

Federal Grant and FQHC Program Support Lead coordination and submission of Federal Section 330 Community Health Center grant requirements; including Service Area Competition, Site-visits, Changes in Scope, Community Health Assessments, and other HRSA requests

Support departments in activities related to FTCA application submission and UDS submissions

Monitor and report on HRSA compliance items and identify risks; including managing forms 5A, 5B, and 5C.Financial and Data Reporting Assist with preparation and coordination of financial documentation related to grants

Work with finance and leadership to ensure alignment between grant budgets and organizational financial practices

Collect, analyze, and report data on grant performance and program outcomes

Cross-Departmental Collaboration Partner with departments such as operations, quality, IT and facilities to support grant-funded initiatives

Assist in conducting market analysis, zip code analysis, including assessing community need, demand, and service gaps Gather success stories, outcomes, and supporting data to strengthen funding opportunities

External Partnerships and Representation Work with external agencies, consultants, and funding organizations to support grant development and compliance

Present grant proposals, findings, and program outcomes to internal and external audiences

Maintain professional relationships with funding entities and community partners

Maintain diverse funding sources through donor cultivation, fund raising campaigns, philanthropy, and special events.

Program and Administrative Support Maintain organized records and documentation for all grant-related activities.

Participate in internal committees, projects, and strategic initiatives as needed

General Expectations Prioritize workload to ensure timely completion of assignments

Demonstrate initiative and accountability in managing responsibilities

Perform other duties as assigned

Minimum Qualifications:

Bachelor’s Degree or a minimum of 5 years of experience in grant writing, grant management, and project development.

FQHC, Healthcare, or nonprofit experience preferred

Required Skills/Abilities:

Outstanding written and verbal communication skills, including grant writing and presentation

Ability to analyze and interpret regulations, funding requirements, and technical documents

Strong organizational and project management skills with the ability to manage multiple deadlines

Strong interpersonal skills and the ability to collaborate across departments and with external partners
