Responsibilities
  • Specialize in post-secondary transition programming for students with disabilities.
  • Develop and implement individualized instruction that promotes independent living, vocational readiness, community engagement, and functional academic skills.
  • Collaborate with families, related service providers, community agencies, and employers to support successful transitions to adulthood, employment, and continued education and training.
  • Facilitate work-based learning experiences, life-skills development, and person-centered planning aligned with students’ IEP goals and transition plans.
  • Assess students’ performance, learning styles, and environments to guide instructional sequences.
  • Develop IEPs based on student assessment data in collaboration with the IEP team.
  • Plan, implement, and evaluate instruction aligned with curricula, student instructional levels, IEP goals, objectives, and specially designed instruction.
  • Provide behavioral support within life-skills and transition programming; monitor and adjust instructional settings to sustain student engagement.
  • Teach learning processes including cognitive strategies; support problem solving, goal setting, and self-evaluation.
  • Adapt expectations, instruction methods, materials, evaluation procedures, and reporting to meet individual student needs.
  • Incorporate district-wide initiatives within lessons; describe student performance using objective data from formal and informal measures.
  • Create a purposeful learning environment with appropriate physical conditions and equipment usage; offer support for physical needs as needed (including coordination with OT/PT).
  • Participate in decision-making teams to coordinate programs addressing physical, social, academic, and community-based integration activities.
  • Act as an educational manager for designated students, monitoring progress per the IEP and facilitating staffings; provide guidance to instructional assistants on instruction and behavior management.
  • Collaborate with teachers, counselors, administrators, families, and community agencies to support student success and post-secondary readiness.
  • Engage with parents/guardians in MDE, IEP, and transition meetings to plan programs and placements; maintain regular communication regarding student progress.
  • Demonstrate leadership to support students in academic, vocational, social, and independent living goals.

  • Provide modeling and coaching in ability awareness to encourage respect and inclusion.
  • Implement activities to foster positive student interactions and social skills development.
  • Use positive, non-aversive methods to support appropriate student behavior.
  • Provide and/or facilitate learning, emotional, and life-skills supports across settings and co-teaching arrangements when appropriate.
  • Reinforce integrated therapies (e.g., OT/PT, Speech/Language) to support skill transfer and generalization.
  • Foster a positive, inclusive, and supportive learning environment that promotes student independence.
General Responsibilities
  • Provide instructional and behavioral support in life-skills and transition programming.
  • Participate in training related to post-secondary transition services and best practices.
  • Coordinate community-based instruction, vocational experiences, and career exploration opportunities.
  • Transport students using a district vehicle as required and maintain a valid driver's license.
  • Experience in life-skills programming with emphasis on post-secondary transition is preferred; strong organizational, interpersonal, and communication skills are essential.
