This position supports patients in Palmyra, PA and surrounding areas and is eligible for a $5,000 first‑year success bonus to reward the accomplishment of key achievements during your first year.

• Collaborate with team members to manage and deliver patient care plans.
• Document care in electronic medical record (EMR) software consistently and accurately.
• Maintain a high level of time and schedule self‑management, including driving up to 20% of the day to patient locations.

• Associate degree in Physical Therapy from an accredited college or technical school.
• Minimum one year of experience in an acute care and/or rehabilitative setting; previous home health experience preferred.
• Valid license/registration as a Physical Therapist Assistant issued by the state in which you work, in good standing.
• Current driver’s license and ability to drive ~20% of the day to/from patient locations.
• Commitment to consistently meet critical deadlines for charting.
• Experience with tablets, mobile phones and EMR software.
• Drug‑screening clearance as part of the background process.
