Home Health Physical Therapist

What You’ll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you.
• Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence.
• Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home.
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation.
• Work independently, but never alone.
• Travel to patient homes within assigned territory.

• Active Physical Therapist, PT license in the state you will work.
• One year of experience as a Physical Therapist and the clinical expertise to deliver quality patient care.
• Current driver’s license and ability to spend approximately 20% of your day driving to/from patient locations.

Full‑time position eligible for $20,000 total bonus potential, including a $10,000 first‑year success bonus and a $10,000 sign‑on bonus paid after 30 days of employment. The bonus opportunities extend beyond the first year: the longer you grow with us, the more your rewards will grow.
