Medical Case Manager – Health Access ServicesMedical Case Managers are members of the interdisciplinary team within the Health Access Services department, serving patients at one or more of Philadelphia FIGHT’s Community Health Center sites. General Overview of KeyRoles & Responsibilities
- Provide support, education, counseling, and guidance to case management staff and consumers during the recovery process.
- Develop and reinforce strengths and goals of clients, and implement interventions consistent with evidence‑based practices.
Ensure that all individuals receive the services they need to live, learn, work, and socialize in the community. Plan and monitor treatment goals, collaborate with family and staff, and coordinate referrals to external resources.
Registered Nurse Case Manager (RN CM) – Serenity Hospice CareRegistered Nurse Case Managers provide compassionate, coordinated care plans, collaborate with interdisciplinary teams, and ensure high‑quality hospice services. They monitor patient status, provide education to families, and support bereavement processes.
Paralegal – Plaintiff Litigation FirmSupport the litigation team by researching asbestos‑related cases, drafting pleadings, managing discovery, and coordinating client communication. Strong organizational skills and attention to detail are essential.
Registered Nurse – Senior Hospice Case ManagerPlan, organize, and direct hospice care with an emphasis on community health education. Develop care protocols, train staff, and maintain compliance with regulatory standards.
Behavioral Health Program Case Manager – RHDProvide trauma‑informed case management support to participants in emergency housing, Safe Haven, and supportive housing programs. Facilitate access to community resources and coordinate case planning meetings.
Sales Case Manager – Care & Help Home CareManage client relationships, assess home care needs, and collaborate with caregivers to ensure quality services. Use performance metrics to drive client satisfaction and retention.
Alternatives to Violence (A2V) Case ManagerProvide direct support, advocacy, case management, and violence‑prevention services to youth and families participating in the A2V program. Develop individualized safety plans and educate families on conflict resolution.
CGRC’s BCM (Blended Case Management)Assist eligible children and families with mental illness to achieve recovery success by blending case management with integrated behavioral health services. Coordinate treatment plans across agencies and monitor outcomes.
Client Services Case Manager – Caring Home CareSupport day‑to‑day home care operations, screen client needs, schedule staff, and manage client records. Ensure regulatory compliance and a high level of client satisfaction.
Recovery Support Outreach Case ManagerEnhance the New Pathways Project by supporting individuals in recovery from substance use disorder. Provide outreach, case coordination, and linkage to treatment and support services.
Registered Nurse – Residential Services TeamEnsure the health and well‑being of individuals with intellectual disabilities in group homes. Oversee medication administration, health monitoring, and collaboration with care staff for individualized care plans.
Accreditation & Licensing Requirements- Graduate of an accredited school of professional nursing required.
- Bachelor’s of Nursing preferred.
- Current and valid license to practice as a Registered Nurse, with specializations in behavioral health or hospice, preferred.
- Masters of Social Work program acceptable for case management roles.
