Job DescriptionElwyn seeks a Community Support Worker to train, counsel, and supervise individuals served in activities of daily living and community inclusion. Responsible for adhering to Individual Service Plan (ISP) goals and documenting progress.
Duties and Responsibilities- Ensure consumer safety in the home and community by adhering to ISP health and safety guidelines and following training guidelines
- Provide supervision, support, and companionship to consumers in the home and community on a one-to-one basis
- Assist consumers in attending medical needs or appointments
- Assist consumers in working toward goals outlined in the ISP and document progress toward those goals
- Perform record-keeping tasks, including submitting time sheets, receipts, personal expenses, and documenting mileage in a timely manner as directed by the supervisor
- Perform other assigned job duties
- Advocate for the disabled individual, communicating needs, self-expression, and goals
- Three (3) years direct care experience
- Must possess a current, valid driver’s license in the state of residence, have a minimum of two (2) years of driving experience within the United States, and have an acceptable driving record
- Demonstrated ability to maintain personal neutrality in challenging situations
- Good interpersonal, written, and verbal communication skills
