DESCRIPTIONOF WORK
This position includes a comprehensive training program designed to equip employees with the knowledge and skills necessary to successfully manage care services for older adults. Training covers the methods, procedures, regulations, and policies that govern the Area Agency on Aging and its programs, while work supports consumers served by Personal Care Homes, Domiciliary Care Homes, and Waivers. As an Aging Care Manager 1, you will participate in the following duties and opportunities:
- Assessment
Activities:
Complete timely annual and bi-annual recertifications and assessments, maintain lists, send advance notices to providers, and help consumers prepare and submit eligibility forms - Care Management:
Develop care plans, link consumers with appropriate service agencies and care placements, enter service details into required systems, and conduct routine visits and monitoring to ensure compliance with regulations and care plan effectiveness - Consumer Support:
Explain regulations, policies, and programs as well as provide counseling on available services to consumers, their families, and community members - Formal and On-the-Job Training:
Participate in individual coaching, group instruction, case reviews, professional reading, training sessions, and ongoing supervision to support your continued growth and success in the role - Professional Development:
As experience is gained and proficiency is demonstrated, employees are assigned progressively more complex responsibilities, provided with increased independence in judgment and decision-making, and earn eligibility for promotions over time

- Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch.

- Three years as an Aging Case Aide 2; or
- A bachelor's degree which includes or is supplemented by twelve (12) credits in social sciences, behavioral sciences, human services, or a closely related field; or
- An equivalent combination of experience and training.
- Applicants will be considered to have met the educational requirements once they are within three months of graduating with a qualifying degree
