, with a primary focus on Jim Thorpe, Lehighton, and surrounding communities.

What You’ll Do- Complete physical therapy evaluations, reassessments, and discharge planning
- Develop individualized treatment plans and provide skilled therapy services in patients’ homes
- Help patients improve strength, balance, mobility, and independence
- Coordinate care with physicians and the interdisciplinary team
- Complete documentation accurately and on time
