FOX Rehabilitation is expanding operations in Philadelphia and Mayfair, PA. We seek licensed Physical Therapists who will deliver exceptional one‑on‑one care to older adults in the comfort of their homes or within senior living communities, using our Geriatric House Calls™ model. This role offers autonomy, flexibility, and the opportunity to manage your own treatment schedule while building strong patient relationships. Telehealth and virtual care options are available to enhance flexibility and continuity of care.

• Provide one‑on‑one physical therapy to older adults in private residences, senior living communities, independent living, assisted living, and memory care settings.
• Perform evaluations, develop individualized care plans, and deliver evidence‑based treatment.
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence.
• Document effectively within FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
• Provide telehealth or virtual care when appropriate to extend access and flexibility.

• Full‑time Physical Therapist – In‑Home & Senior Living Care (salaried with benefits)
• Part‑time Physical Therapist – In‑Home & Senior Living Care (salaried with benefits; eligibility based on minimum hours)
• PRN / FOX Flex Physical Therapist – Home & Community Visits (pay per unit compensation)

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain one.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

• Must be able to lift 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly.
• Perform CPR and provide moderate to maximum assistance with patient transfers and bed mobility.
• Reposition patients, stand/walk for extended periods, climb stairs, and perform therapeutic activities.
• Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
• Wearing PPE when required.
