Community LIFE provides services for nursing home eligible adults aged 55+ living in the community. As a Home Care Coordinator (HCC), you will work with the Interdisciplinary Team to assess participant needs and coordinate delivery of participant-centered home care services.
The role focuses on maximizing rehab potential in activities of daily living and minimizing inpatient care, supporting transitions to home after hospitalization and during rehab. Weekend/on-call rotation is required.
