Assist with showering, cleaning, and taking clients out in the community.;
Assist the Individual and the interdisciplinary team in developing and maintaining the Individual’s ISP (Individual Support Plan).;
Complete documentation (EVV, daily service note).;
Immediately reports any changes or problems in the Individuals condition to manager.;
Assist individuals in living an everyday life and developing life skills.
