Drug And Alcohol Case Management Worker

The Drug and Alcohol Case Management Worker provides case management services to individuals experiencing substance use disorders (SUD). This position involves assessing client needs, developing individualized service plans, coordinating treatment, and connecting individuals with appropriate resources, such as rehabilitation programs, housing, and employment assistance. The case manager acts as an advocate for clients, ensuring they receive the necessary services to support their recovery journey.

Essential Duties And Responsibilities

  • Conduct initial intake assessments and determine clients' level of care needs.
  • Develop individualized service plans (ISPs) based on client needs and goals.
  • Work closely with the local SCA and navigate the PA WITS program
  • Monitor client progress in treatment and adjust plans accordingly.
  • Coordinate referrals to rehabilitation centers, outpatient programs, support groups, housing services, and other community resources.
  • Work closely with treatment providers, social workers, probation officers, and other stakeholders to ensure continuity of care.
  • Maintain accurate and confidential records in compliance with state and federal regulations.
  • Educate clients and families on substance abuse, treatment options, and recovery resources.
  • Assist clients with securing employment, housing, and other social services.
  • Have the ability to input/coordinate authorizations for insurances, SCA and treatment court.
  • Advocate for client needs within the legal and healthcare system.
  • Participate in staff meetings, case reviews, and professional development opportunities.

Qualifications

Previous work experience in drug and alcohol counseling, social services, or case management is preferred. Knowledge of Pennsylvania's drug and alcohol treatment system, Medicaid regulations, and local community resources. Strong communication, problem-solving, and organizational abilities. Ability to handle sensitive information confidentially and work with diverse populations.

Education and/or Experience

1. A Bachelor's Degree from an accredited college with a major in chemical dependency, psychology, social work, counseling, nursing (with a clinical specialty in the human services) or other related field and 1 year of clinical experience (a minimum of 1,820 hours) in a health or human service agency, preferably in a drug and alcohol setting or;

2. An Associate Degree from an accredited college with a major in chemical dependency, psychology, social work, counseling, nursing (with a clinical specialty in the human services) or other related field and 2 years of clinical experience (a minimum of 3,640 hours) in a health or human service agency, preferably in a drug and alcohol setting.

3. Knowledge of drug abuse subcultures

4. Staff requirements for certification; HIV,STD, ethics, etc.

5. Knowledge of treatment facilities and human service agencies providing services to drug and alcohol dependent clients.
