** General Summary*
* Provides pharmaceuticals and drug information as needed for adequate patient care according to professional standards and practices.

** Responsibilities*

* Essential Functions:

* + Compounds pharmaceutical preparations and/or labels and packages medications and pharmaceuticals. Assists with and/or supervises IV preparation and/or delivery.

+ Advises patients and medical/nursing staff on use of various medications including dosage, side effects, composition, drug-to-drug interactions, drug-to-allergy interactions and drug-to-food interactions.

+ Ensures proper controls and records of narcotics and other controlled substances as required by state and federal law.

+ Monitors drug therapy to identify, prevent or resolve actual or potential drug therapy problems. Addresses problems with physicians and/or nurses when indicated.

+ Provides clinical pharmacy services as needed.

+ Administers immunizations, reviews patient profiles for immunization qualification and makes that recommendation to the patient.

+ Assists with and/or supervises personal care/long term care medication delivery.

+ Assists in preparing required reports by compiling information.

+ Authorization to Administer Injectables for Pharmacist Upon Hire Preferred
