Home Care Evolution - - Responsibilities:
Provide personalized daily support to residents including hygiene, dressing, and grooming;
Create sensory-friendly environments with routines to support individuals with sensory processing needs;
Provide behavioral and emotional support with patient redirection;
Prepare healthy meals and snacks and assist with feeding as needed;
Light housekeeping and organization to maintain a safe, calm home environment;
Drive residents in a company-provided vehicle to community events and doctor's appointments;
Health advocacy including medication reminders and monitoring health or behavioral changes
