Home Health Physical Therapist

This home health Physical Therapist role is based in Selinsgrove, Pennsylvania, a welcoming Central Pennsylvania community known for its small-town charm, friendly neighborhoods, and easy access to the Susquehanna River. In this position, you will provide one-on-one, patient-centered physical therapy services in the home setting, supporting primarily adult and geriatric patients recovering from illness, surgery, or managing chronic conditions. You will conduct comprehensive evaluations, develop and implement individualized care plans, and deliver skilled interventions focused on mobility, strength, balance, endurance, and safety in the home.

Typical responsibilities include performing initial and follow-up assessments in patient homes, establishing functional, measurable goals, delivering therapeutic exercise, gait training, transfer training, and balance programs, educating patients and caregivers on home exercise programs and safe mobility, collaborating closely with an interdisciplinary home health team, and documenting all care in the electronic medical record in accordance with regulatory standards.

A typical day involves traveling within Selinsgrove and neighboring communities to complete a set number of home visits, prioritizing quality of care and safety. You can expect a reasonably manageable caseload designed to allow for thorough assessments, direct treatment, and timely documentation. Visit volumes and schedules are designed to provide a balance between autonomy and support, with clinical leadership and office-based staff available as needed.

You may encounter a varied caseload that includes post-acute orthopedic cases (such as joint replacements), cardiopulmonary diagnoses, general deconditioning, neurologic conditions, and patients at risk for falls. There is opportunity to refine your skills in home safety evaluations and adaptive equipment recommendations. The home health setting offers a strong sense of professional independence while still supporting team-based care and communication.
