Provide one-on-one 60;physical therapy 60;to older adults wherever they call home (Private 60;residences, Senior Living Communities, Independent Living, Assisted Living, Memory Care) 60;
Perform evaluations, develop individualized plans of care, and deliver 60;evidence-based 60;treatment 60;
Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence 60;
Document effectively within FOX’s EMR system 60;in accordance with 60;regulatory standards 60;
Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists 60;
Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place 60;
Provide telehealth / virtual care when 60;appropriate to 60;extend access and flexibility 60;

Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
Degree from an accredited
Physical Therapy programBasic computer and EMRproficiency
Current CPR certification
FOX clinicians must be able to meet the physical demands ofin-homeandcommunity-basedcare, including:
Exerting medium physical effort, including lifting 25–50lbsoccasionally, 10–25lbsfrequently, and up to 10lbsconstantly
Performing CPR
Providing moderate to maximumassistancewith patient transfers and bed mobility
Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities
Driving to patient homes and senior living communities as part of the Geriatric House Calls™ model
Wearing PPE when required
