Chester County Department of MH/IDD seeks an Early Intervention Service Coordinator to facilitate, coordinate and monitor services for children birth to three with delays. The role includes home visits, community meetings, and attending trainings throughout the year.
The coordinator provides case management for at-risk/with delays, develops plans, and maintains case records and progress notes. Duties vary by caseload and may include intake meetings and IFSP support as needed.
