Home Health Nurse – Home Health

8:00 AM - 4:30 PM

Assignment Details:

Requirements:

• Recent and relevant home health experience
• EPIC experience
• OASIS certification required

Roles & Responsibilities:

• Conduct patient assessments and develop care plans.
• Provide in-home patient care and support.
• Perform start of care and revisit visits.
• Document patient progress and ensure compliance with regulations.
• Educate patients and families on health management and care needs.
• Collaborate with healthcare teams to optimize patient outcomes.
