Role Overview

An Occupational Therapist in Home Health assesses and evaluates patients  functional status, daily activities, fine motor skills, home environments, and adaptive equipment needs, developing personalized therapy plans in collaboration with physicians and the healthcare team.

• Providing services within legal scope, using evidence-based techniques, and coordinating with team members
• Evaluating patient function and health status for documentation and care planning
• Monitoring and reporting patient responses and condition changes
• Conducting assessments to determine functional ability using diagnostic tests

• Current Occupational Therapy license and CPR certification
• Valid driver’s license, vehicle insurance, or access to transportation
• Ability to work effectively with diverse patient populations in home settings

• Multitasking and independent work capabilities
• Strong communication, organizational, and writing skills
