Role

Occupational Therapist in Home Health, providing assessment, evaluation, and therapy services to patients in their homes.

• Assess and evaluate patients' functional status, daily activities, and need for adaptive equipment.
• Develop and implement individualized treatment plans in collaboration with physicians and healthcare team.
• Conduct patient assessments using diagnostic and prognostic functional tests.
• Monitor and document patient responses to treatment and report changes to supervisors and physicians.
• Perform home assessments ensuring safety and suitability for therapy activities.
• Provide care regardless of patient age, race, creed, or socioeconomic status, within scope of practice and legal guidelines.

• Current state licensure in Occupational Therapy.
• Valid driver's license and reliable transportation or access to public transit.
• Preferred: CPR certification, ability to manage multiple tasks independently, strong communication, organizational skills.
