POSITION OVERVIEW:
This position is responsible for planning, developing and administering occupational therapy services to patients with a variety of diagnoses with the goal of facilitating the resumption of the patient’s prior life roles through remediation or adaptations to achieve their maximum level of functioning.
ESSENTIAL FUNCTIONS OF JOB: (other duties may be assigned)
Perform patient evaluations.
Daily, conduct patient evaluations including all pertinent tests and assessments of activities of daily living, work and productive/leisure activities, sensorimotor, cognitive skills and psychosocial skills.
Complete evaluations on inpatients within 24 hours of receipt of the order from the referring physician.
Outpatient evaluations must be initiated within 15 minutes of scheduled appointment.
Write and complete the evaluation results and establish an appropriate plan of care.
Inpatient evaluations must be written by 4:00 p.m. of the day the evaluation is performed.
Outpatient evaluations and home health evaluations must be written and completed within two working days of the date the evaluation is performed.
Establish the plan of care with patient oriented goals that are measurable.
Write therapy plans and progress reports.
Daily write therapy plans and progress reports to indicate progress towards established goals, patient response to treatment, change in symptoms and measurements.
Utilize SOAP format in evaluations and daily progress notes to indicate the patient’s status.
Perform re-evaluations and write the results every 30 days.
Write discharge summaries and forward to physician within two days of patient discharge.
Perform occupational therapy treatments.
Daily administer therapeutic and purposeful activities in daily living, work and productive activities, sensorimotor, cognitive and psychosocial areas to patients in an age appropriate and safe manner.
Recognize indications, contraindications and precautions to above areas.
Perform extensive chart reviews to recognize any contraindications and necessity of occupational therapy services.
Modify patient treatment program when necessary.
Instruct patients and caregivers of appropriate interventions, adaptive equipment, splints and braces.
Explain to patient and caregivers the purpose, use and necessity of adaptive equipment, splints and braces, compression supplies
Provide handouts of used interventions, exercises and provide a schedule of when to perform.
Obtain patient and caregiver feedback to measure understanding of the explanation and instruction provided.
Ensure that occupational therapy services are delivered in accordance with established standards.
Daily, strictly adhere to American Occupational Therapy Association guidelines, department and hospital policies in providing occupational therapy to patients.
Evaluate and treat patient only if a referral from a licensed physician is supplied.
Maintain strict patient confidentiality.
Provide in-service training for rehab staff on a yearly basis
Provide correct and appropriate information on in-service topic
Training is to last 30 minutes with proper visual aides
Schedule patients, prepare daily route sheet, and bi-weekly mileage form Schedule patient appointments at least 1 day in advance
Mileage form completed and turned in every two weeks
Route sheet turned in daily
Remain complaint with standard precautions
Demonstrate proper bag technique
Demonstrate proper hand hygiene
Demonstrate proper Respiratory hygiene and cough etiquette
Follow infection prevention policies and procedures
Submit and access patient claim information/physical therapy eligibility through Navinet system for reimbursement by Highmark Blue Cross Blue Shield Enter appropriate diagnostic (ICD
10) code as well as patient’s demographic information through Navinet system within 24 hours of initial occupational therapy evaluation
Obtain hard copy of patient’s physical therapy approval, eligibility, and benefits and place hard copy into patient’s medical chart
Assist with Performance Improvement Program Compile data on current PI project
