The Program Specialist plays a pivotal role in developing and implementing programs and services that advance our mission to support individuals who are deaf and/or hard of hearing and also have additional intellectual and developmental disabilities. The Program Specialist will work closely with individuals to understand their aspirations and support them in achieving their goals. This includes coordinating activities that align with person-centered planning and ensuring health and safety needs are met.
Proficiency in American Sign Language preferred.
- Adhere to ODP guidelines and PA codes for 6400 regulations.
- Facilitate and troubleshoot program activities, ensuring high-quality homecare operations.
- Monitor residential homes to ensure they are a safe, homelike environment that reflects individual preferences.
- Complete assessments and participate in the development and implementation of Individual Support Plans (ISPs), Behavior Support Plans (BSPs), and other care strategies.
- Develop, manage, and evaluate programs to meet the unique needs of our individuals.
- Collaborate with internal teams and external partners to identify program gaps and opportunities for improvement.
- Build and maintain relationships with community partners and stakeholders.
- Collect and analyze data to measure program outcomes and identify areas for enhancement.
- Provide support to individuals as needed.
- Stay informed on the latest research and best practices in program development and management.
- Coordinate medical appointments and oversee aspects of medical care.
- Ensure that all reporting and assessments are accurate, timely, and compliant with requirements (including Program Books, Quarterly Reports, Annual Assessments, Lifetime Medical History, ISP, BSP, MARS, daily notes, and monthly paperwork).
- Facilitate the development of a cohesive team focused on individual support.
- Provide training and model support strategies to ensure successful implementation.
- Assign team roles to ensure smooth operations in caring for individuals.
- Ensure compliance with legal and regulatory requirements.
- Facilitate monthly Supports Coordinator monitoring visits.
- Attend weekly Direct Support Supervisor meetings.
- Support individual relationships and communications within the program.
- Participate in the on-call rotation
- Ensure staff are trained on ISP goals and outcomes.
- Maintain working knowledge of software and tools required to implement policies and procedures effectively.
- Perform other duties as assigned.
This position has no supervisory responsibilities.
Minimum Qualifications:- Bachelor's degree in social work, education, or a related field.
- At least 2 years of experience in program development and management.
- Strong organizational and time management skills.
- Ability to thrive in a fast-paced environment and manage multiple priorities.
- Proficiency in American Sign Language preferred.
- Must be able to regularly lift and carry up to 20 lbs. and occasionally pull at least 40 lbs.
- Must be able to squat, kneel, crawl, crouch, climb, and stoop as part of regular job duties.
- Must be able to use hands and fingers to handle or operate objects, tools, or controls.
- Required to stand and walk for extended periods.
- Must be able to demonstrate proficiency in CPR from floor level, which requires working on hands and knees, bending, standing, and lifting.
- Vision requirements include close, distance, and peripheral vision.
- Must be able to talk and hear.
- Exception – Deaf and Hard of Hearing Programs:
For these programs only, employees must be fluent in American Sign Language (ASL). The requirement to talk and hear does not apply.
