Medical Social Worker

Lehigh Valley Health Network seeks a compassionate Medical Social Worker to support patients and families across the care continuum. In this hospital-based role, you'll complete psychosocial assessments, provide counseling and crisis intervention, and coordinate discharge plans and community resources. Working within LVHN's collaborative culture, you will partner with interdisciplinary teams to address social determinants of health, reduce barriers to care, and promote patient well-being in eastern Pennsylvania.

Responsibilities

• Conduct comprehensive psychosocial assessments for patients and families.
• Develop and implement patient-centered care and discharge plans.
• Provide counseling, crisis intervention, and advocacy for patients and caregivers.
• Collaborate with physicians, nurses, and interdisciplinary teams to coordinate care.
• Connect patients with community resources, financial assistance, and support services.
• Document assessments, interventions, and outcomes in the EHR in a timely manner.
• Support care transitions to home, rehab, or long-term care facilities.
• Participate in quality improvement initiatives and case conferences.
• Educate patients and families about coping strategies and available services.
• Uphold LVHN standards of compassion, ethics, and cultural sensitivity.
