Lehigh Valley Health Network is seeking a Clinical Team Coordinator to support interdisciplinary care teams across our eastern Pennsylvania network. In this role, you will coordinate patient flow, schedules, and communications between clinicians, clinics, and hospitals to ensure safe, timely, and patient-centered care. You will track care plans, manage documentation, support quality and safety initiatives, and help onboard new team members.
Ideal candidates bring strong clinical knowledge, EMR proficiency, organizational excellence, and a collaborative, compassionate approach aligned with LVHN's mission and values.
Responsibilities
- Coordinate daily clinical team schedules, patient flow, and resources across clinics and hospital departments.
- Serve as a central communication hub between physicians, nurses, allied health, and administrative staff.
- Monitor and track patient care plans, follow-up appointments, and referrals to support continuity of care.
- Maintain accurate EMR documentation and ensure timely entry and retrieval of clinical information.
- Support quality improvement, patient safety, and performance metrics through data tracking and reporting.
- Assist with onboarding, training, and supporting new clinical team members on workflows and systems.
- Ensure compliance with HIPAA, organizational policies, and applicable healthcare regulations.
- Collaborate with leadership to identify workflow gaps and implement process improvements.
