Job Summary:

Seeking a licensed Physical Therapist to provide specialized outpatient therapy services. The ideal candidate will focus on patient rehabilitation and functional recovery through individualized treatment plans.

Responsibilities:

• Evaluate patients' physical conditions to determine appropriate treatment strategies.
• Develop and implement personalized therapy plans to improve mobility and reduce pain.
• Monitor patient progress and adjust treatments accordingly.
• Document all patient assessments, treatments, and outcomes accurately.
• Educate patients and caregivers on exercises and home programs to support recovery.
• Collaborate with healthcare teams to ensure comprehensive patient care.

Qualifications:

• Valid state license as a Physical Therapist.
• Proven experience in outpatient or rehabilitation settings.
• Strong interpersonal and communication skills.
• Ability to assess patient needs and develop effective treatment plans.
