As a Physical Therapist at Keystone Care, you will make a meaningful impact by helping patients strengthen mobility, restore function, and maintain independence through compassionate in-home care. This role is ideal for someone who is energized by one-on-one patient support and collaborative care, and who wants to contribute to better outcomes for individuals and families across the communities we serve.
Responsibilities- Provide personalized rehabilitation services to patients in their homes.
- Assess, plan, and carry out treatment programs focused on improving mobility, strength, and overall functional abilities.
- Collaborate closely with a multidisciplinary team to support patient care and recovery.
- Guide patients through their recovery journey with compassion and professionalism.
- Help restore patients' independence and well-being through high-quality therapeutic care.
- Graduate of an accredited Physical Therapy program
- Current Pennsylvania Physical Therapy License
- Minimum two years of physical therapy experience in Home Health or similar settings
- Experience treating geriatric patients in a home health or similar care setting
- Experience with post-surgical rehabilitation, neurological conditions, and orthopedic rehabilitation in a home health or similar care setting
- Experience providing care in hospice-adjacent contexts
- Valid Driver’s License and reliable vehicle
- Proof of Auto Insurance
- Strong communication skills for effective collaboration with patients, families, and the care team
- Excellent organizational abilities and experience managing a diverse caseload
- Eligible to work in the United States
- Experience with Matrix Care is a plus.
