Medical Social Worker; MSW; Home Care

Role Overview: The Medical Social Worker (MSW) in Home Care provides psychosocial assessment, counseling, crisis intervention, and resource referral services to patients and families. They collaborate with management to optimize resource utilization for positive patient outcomes.

• Conduct psychosocial evaluations and develop individualized care plans.
• Employ counseling techniques to help patients/families understand and follow care recommendations, promoting social and health adjustment.
• Intervene effectively in crisis situations using appropriate techniques.
• Identify and document barriers to care, including clinical, psychosocial, and financial challenges.
• Stay informed on community resources for social, emotional, and financial support.
• Participate in audits to ensure compliance with laws and guidelines.
• Report suspected abuse or neglect cases.
• Work collaboratively with patients and support systems to establish care plans.
• Interview patients to complete psychosocial assessments.

• Master’s Degree in Social Work; one year of relevant experience.
• Effective communication skills and cultural competence.
• Adherence to the NASW Code of Ethics and knowledge of patient rights and laws like HIPAA.
• High emotional intelligence and empathy.
• Logical reasoning to analyze solutions and approaches.
• Preferred:
  State of Pennsylvania Licensed Social Worker (LSW).

Physical & Other Requirements: Ability to lift 7 lbs., sit for extended periods, and perform repetitive motions.
