Home Care Aide
UPMC Family Hospice is hiring a Home Care Aide for the Wellsboro area, covering Tioga, Bradford, and Potter.
Monday-Friday 8:00 am-4:30 pm
Two holidays a year
Weekend Rotation (Saturdays approx. every 4th/5th weekend)
Purpose:
Provide designated care under the direction of the primary service provider in the home in order to achieve identified goals. Function as an extension of skilled nursing services and/or rehabilitation services in the home.
Responsibilities:
- Must maintain annual competency evaluation scores as determined by regulatory requirements
- Attends compliance training and adheres to the organization's standards of conduct, policies, and procedures.
- Proven ability to facilitate change.
- Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction.
- Identifies, develops, and participates in process improvement opportunities within the home health agency that will enhance the quality of services we provide.
- Performs duties and job responsibilities in a fashion that coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians, and other departments within the organization.
- Participates in all required agency meetings.
- Communicates appropriately with the interdisciplinary home care team promptly, reporting any changes in the patient's condition to appropriate professional staff.
- Maintains and completes documentation accurately and promptly while in the patient's home and in accordance with agency and regulatory standards.
- Provides safe and direct personal care to patients in the home setting under direct supervision of an appropriate professional.
High School Diploma or GED
- Required
Hands-on direct patient personal care in the last 5 years preferred
One (1) year of experience
- Preferred
Certified Nurse Aide – Preferred
Minimum of 75 hours of Nurse Aide training – Required (If no previous nurse aide training, applicant may attend UPMC Home Healthcare Nurse Aide Training Course as available upon hire.)
Licensure, Certifications, and Clearances:
- Automotive Insurance
- Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
- Driver's License
- Act 33
- Act 34
- OAPSA
