Medical Social Worker (MSW) in Home Care.

• Conduct psychosocial assessments, counseling, crisis intervention, and provide information/referral services.
• Collaborate with management to optimize resource utilization for patient outcomes.
• Assist patients and families in understanding and accepting care recommendations to promote social and health adjustment.
• Stay informed on community resources for social, emotional, and financial support.
• Identify and document barriers to healthcare transition, including clinical, psychosocial, and financial issues.
• Participate in audits to ensure compliance with laws and guidelines.
• Work with patients and support systems to develop and implement care plans.
• Perform initial psychosocial evaluations through patient interviews.
• Report suspected cases of abuse or neglect.

• Master’s Degree in Social Work.
• Minimum 1 year of social work or mental health experience.
• Effective communication with diverse populations and adherence to NASW Code of Ethics.
• Knowledge of patient rights, laws (like HIPAA), and psychological principles.
• High emotional intelligence, empathy, and reasoning skills.
• Preferred: LSW license in Pennsylvania.

Ability to work collaboratively, strong ethical standards, and proficient in crisis intervention and resource navigation.
