Physical Therapist

Seeking a dedicated and compassionate Physical Therapist (PT) to provide high-quality rehabilitation services in a skilled nursing environment. The PT will evaluate residents, develop individualized treatment plans, provide therapeutic interventions, and collaborate with an interdisciplinary care team to help residents achieve their highest level of functional independence.

• Perform comprehensive physical therapy evaluations and functional assessments.
• Develop and implement individualized treatment plans based on residents’ clinical and functional needs.
• Provide therapeutic exercises, gait training, balance training, transfer training, and mobility interventions.
• Monitor resident progress and modify treatment plans as appropriate.
• Maintain accurate and timely clinical documentation, treatment records, and progress notes.
• Participate in interdisciplinary care planning and team meetings.
• Collaborate with physicians, nurses, OTs, SLPs, CNAs, and other healthcare professionals.
• Educate residents and families on therapeutic exercises, mobility, safety, and home exercise programs.
• Recommend appropriate assistive devices, positioning, transfer techniques, and fall-prevention strategies.
• Participate in discharge planning and support safe transitions of care.
• Maintain compliance with facility policies, professional standards, and applicable state and federal regulations.

• Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program.
• Current Pennsylvania Physical Therapist (PT) license

• Strong clinical evaluation, treatment, and documentation skills.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a multidisciplinary healthcare environment.
