Taconic at Hopewell is seeking a compassionate and skilled Part‑Time Speech‑Language Pathologist (SLP) to join our interdisciplinary rehabilitation team. Position Summary

The Speech‑Language Pathologist provides evaluation, diagnosis, and treatment of speech, language, cognitive, and swallowing disorders for residents in a skilled nursing and rehabilitation setting. The SLP collaborates with the care team to improve safety, independence, and quality of life.

Key Responsibilities
  • Evaluate and treat residents with speech, language, cognitive, and swallowing disorders
  • Develop and implement individualized care plans
  • Perform dysphagia assessments and recommend safe dietary modifications
  • Collaborate with nursing, dietary, and rehabilitation teams
  • Educate residents, families, and staff on communication and swallowing strategies
  • Maintain accurate documentation in accordance with regulatory standards
  • Support quality measures and patient outcome goals
Qualifications
  • Degree in Speech‑Language Pathology
  • Current SLP license (required)
  • Certificate of Clinical Competence (CCC‑SLP) preferred
  • Experience in skilled nursing or post‑acute care preferred
  • Strong clinical, communication, and documentation skills
