Job Summary
Homeside Rehab is seeking compassionate and skilled Occupational Therapists (OTs) and Physical Therapists (PTs) to join our home health team in Spring Grove, PA
.
In this role, you will provide individualized, evidence-based therapy services to patients in the comfort of their homes. This is a great opportunity for therapists who value schedule flexibility, one-on-one patient care, and a supportive clinical and administrative team.
Job ResponsibilitiesEvaluate patients and develop individualized treatment plans based on clinical assessments, functional needs, and patient goals
Provide skilled Occupational Therapy or Physical Therapy interventions within your professional scope of practice
Help patients improve mobility, strength, motor skills, functional independence, and overall quality of life
Monitor patient progress and adjust treatment plans as clinically appropriate
Educate patients and caregivers regarding treatment goals, home exercise programs, safety, and functional strategies
Maintain accurate and timely clinical documentation in accordance with company and regulatory requirements
Collaborate with the clinical team to ensure coordinated and effective patient care
Communicate professionally with patients, caregivers, office staff, and other members of the care team
Active Pennsylvania Occupational Therapist (OT) or Physical Therapist (PT) License – Required
Basic computer proficiency for electronic documentation
Strong clinical assessment and interpersonal communication skills
Ability to work independently in a home health environment
Valid driver's license and reliable transportation for home visits
Pediatric experience preferred
Minimum of 1 year of experience in Home Health, Outpatient, Rehabilitation, or Skilled Nursing Facility (SNF) settings preferred
Job Type: Full-Time, Part-Time, Per Diem
Location: Spring Grove, PA
Pay: $65–$80 per hour
Schedule: Flexible – choose your own availability
Day and evening shifts available
Monday through Friday
Weekend availability as needed
