Occupational Therapist, Home Health

Role Overview :

The Occupational Therapist in Home Health assesses and evaluates patients' functional status, activities of daily living, fine motor skills, and home environment needs to develop and implement personalized treatment plans.

• Provide occupational therapy services within legal scope, following the plan of care and using evidence-based techniques.
• Evaluate patients' functional abilities, document CMS-required data, and collaborate with physicians on therapy plans.
• Monitor and report patient responses to treatment and any condition changes.
• Perform assessments to determine functional levels using diagnostic and prognostic tests.
• Adapt to diverse home environments and client backgrounds, ensuring effective care delivery.

• Current state licensure in Occupational Therapy.
• Valid driver’s license, vehicle insurance, or access to reliable transportation.
• Preferred: CPR certification, multitasking, independence, strong communication, organizational, and documentation skills.
