We are seeking a licensed Physical Therapist to provide one-on-one care to older adults in their homes and senior living communities, utilizing our Geriatric House Calls™ model.
Key responsibilitiesinclude conducting evaluations, developing individualized treatment plans, delivering evidence-based therapy, documenting in EMR, collaborating with interdisciplinary teams, and educating patients and caregivers to promote safety and independence. Telehealth and virtual care are incorporated to enhance flexibility and care continuity.
Qualificationsrequired are a valid PT license or eligibility, degree from an accredited program, basic computer skills, and CPR certification. Candidates must meet physical demands such as lifting, assisting with patient transfers, and driving to patient locations.
