Physical Therapist Assistant (PTA) — PRN

Chestnut Hill Lodge Health & Rehab Center is seeking a PRN Physical Therapist Assistant (PTA) to provide flexible, high-quality rehabilitation support.

The PTA will carry out treatment plans developed by the Physical Therapist and help residents improve mobility, safety, and functional independence.

• Provide skilled therapy interventions under the supervision of a Physical Therapist.
• Assist with exercises, gait training, transfers, balance activities, and functional mobility.
• Observe resident response to treatment and report progress or concerns.
• Complete accurate and timely documentation.
• Collaborate with the rehabilitation and interdisciplinary care teams.

• Current PTA license or eligibility to practice in the applicable state.
• Graduate of an accredited Physical Therapist Assistant program.
• Skilled nursing, long-term care, or rehabilitation experience preferred.
• Dependable, flexible, and resident-focused approach.

PRN / as needed.
