Performs hands‑on activity programming with residents in large, medium, small group and one‑to‑one settings, assessing and fulfilling their psychosocial, emotional and recreational needs.

• Plan, organize and implement activity programs and special events for the Personal Care, Skilled Nursing Unit, and the Dementia Program according to the department schedule and assignments including evening, weekend, holiday, indoor and outdoor activities.
• Design programs that meet residents’ psychosocial, emotional and recreational needs.
• Provide sensory stimulation for residents at risk of sensory deprivation due to dementia progression.
• Communicate with charge nurses and team members regarding residents’ conditions, requests and interests.
• Plan and implement one‑to‑one activities for residents who require individualized programming.
• Maintain required documentation for departmental records, attendance, medical records and care plans as assigned by the Activity Director.
• Participate in completing quarterly progress note charting.
• Use reality orientation and reminiscence therapy appropriate to the resident’s cognitive function in group or one‑to‑one settings.
• Ensure safety of residents under supervision at all times, including bus trips, community events, and outings.
• Supervise volunteer participation in group activities when assigned.
• Monitor emotional and psychosocial needs and provide interventions as necessary.
• Plan for supplies and equipment to be available during programming; observe infection control procedures related to activity equipment.
• Provide nutritional snacks in coordination with Dietary Services, considering residents’ swallowing abilities.
• Purchase departmental supplies as directed.
• Place residents at dining tables at meal time.
• Put clothing protectors on residents before meals and feed residents.
• Make decorations for special theme events.
• Post calendars in resident rooms and hand out resident council minutes and baseball schedules.
• Clean closets and supply cupboards to keep them orderly.
• Escort residents to and from large group activities; introduce and thank entertainers.
• Assist as needed for special events such as Safe Halloween, National Nursing Home Week and Christmas Party.
• Immediately report resident related care concerns to the Department Supervisor and Executive Director.
• Interact with team members, residents and guests courteously and professionally.
• Keep information confidential in accordance with policies and the Team Member Handbook.
• Understand and uphold Resident Rights.
• Follow each resident’s Plan of Care and oral and written instructions.
• Complete assigned tasks per organization, facility and department procedures.
• Work independently and as part of a team.
• Use supplies efficiently and cost effectively.
• Treat equipment with proper care and use it only for its intended purpose.
• Report equipment problems to maintenance according to facility guidelines.
• Apply infection control practices, including standard precautions.
• Answer call bells and assist as able.

• Interact effectively with residents, coworkers and guests in a respectful, professional manner and maintain a calm demeanor in difficult situations.
• Follow company and departmental policies, including safety, privacy, and regulatory requirements.
• Complete work tasks thoroughly, accurately and efficiently; work without close supervision; arrive on time and accept schedule changes or additional hours.
• Represent the company positively in all communications with residents, families, suppliers and the community.
• Follow safety policies, use provided safety equipment, and identify potential safety hazards.

• At least two years of activity experience.
• Activities certification or be a qualified occupational therapist.
