Residential Home Health - - Responsibilities:
Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life;
Provide hands-on care to patients, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support comfort and quality of life;
Support and guide patients and caregivers by reinforcing the care plan, offering emotional support, and helping them understand what to expect during the hospice journey;
Respond promptly to patient needs with empathy and attentiveness and accurately document care provided in the EMR;
Travel to patient homes within assigned territory
