As a Physical Therapist at Pocopson Home
, you'll evaluate and treat patients with a variety of mobility and functional needs while developing individualized treatment plans based on each person's abilities and goals.

Your role will include:

• Performing comprehensive Physical Therapy evaluations
• Developing individualized plans of care and functional goals
• Addressing strength, balance, gait, transfers, endurance and functional mobility
• Providing therapeutic exercise and functional mobility training
• Developing fall-prevention and safety strategies
• Assessing the need for walkers and other assistive devices
• Monitoring patient progress and modifying plans of care when appropriate
• Supervising and collaborating with Physical Therapist Assistants
• Educating patients, families and caregivers
• Collaborating with the interdisciplinary care team
• Completing accurate and timely clinical documentation
