• Active RN license in practicing state required.
• Experience in SNF, LTC, or rehab preferred.
• Current BLS required; ACLS may be required.
• Strong assessment and critical-thinking skills.
• Knowledge of SNF regulations and infection control.
• Ability to provide compassionate, patient-centered care.

• Patient Care & Assessment:
  • Assess and monitor patient conditions regularly.
  • Administer medications, IV therapy, and treatments.
  • Develop and implement individualized care plans.

• Documentation & Compliance:
  • Document all patient care and progress in the EMR.
  • Ensure patient safety and compliance with policies.
  • Collaborate with interdisciplinary team members.

• Education:
  • Educate patients and families on care plans and health.
