Concordia Hospice of Washington is a partnership between Concordia Community Support Services and Washington Health System that offers both quality home care and inpatient hospice end-of-life care. Concordia Hospice of Washington serves our patients in Washington and Greene counties with the focus to provide services that offer comfort, peace, and dignity for those with incurable illnesses and for their families. Concordia Hospice of Washington is seeking a compassionate, relationship-driven Registered Nurse (RN) to join our team in a dual Admissions / Liaison role. This position is ideal for a nurse who enjoys both clinical care and building strong partnerships within the healthcare community.
Responsibilities- Admissions Responsibilities
:
Conduct hospice admissions in the home setting. - Perform comprehensive patient assessments.
- Educate patients and families on hospice services and care plans.
- Coordinate with the interdisciplinary team to ensure a smooth transition to care.
- Deliver compassionate, patient-centered interactions during a critical time.
- Liaison Responsibilities
:
Develop and maintain relationships with referral sources. - Work closely with partnered hospitals and healthcare facilities.
- Facilitate and receive patient referrals.
- Serve as a trusted resource on hospice eligibility and services.
- Represent Concordia Hospice of Washington with professionalism and integrity.
Important: Must be a Registered Nurse (RN) – required to perform hospice in-home admissions.
