Concordia Hospice of Washington is a partnership between Concordia Community Support Services and Washington Health System that offers both quality home care and inpatient hospice end-of-life care. Concordia Hospice of Washington serves our patients in Washington and Greene counties with the focus to provide services that offer comfort, peace, and dignity for those with incurable illnesses and for their families. Concordia Hospice of Washington is seeking a compassionate and dependable On-Call Licensed Practical Nurse (LPN) to provide comfort-focused care and support to patients and families outside of regular business hours. This role is ideal for a nurse who is confident working independently and committed to delivering meaningful, patient-centered care.

This position requires travel throughout the South Hills region and Washington County.

• A Licensed Practical Nurse with a current, active license
