Location: Huntingdon Valley

Registered Nurse Case Manager (RNCM) – Hospice

Location: Pennsylvania – Patient Residences (Travel Required)
Schedule: Full-Time | Monday–Friday, 8:00 AM–5:00 PM | Occasional evenings/weekends
Salary: $85,000–$100,000/year (Above Industry Standard)

Your Role as a Registered Nurse Case Manager (RNCM)

As an RNCM, you’ll be a cornerstone of our interdisciplinary team, ensuring patients receive personalized, compassionate care. You will:

  • Conduct patient admissions and comprehensive nursing assessments.
  • Develop and implement individualized plans of care.
  • Collaborate with hospice physicians, social workers, chaplains, and aides.
  • Educate patients and families on symptom management and available resources.
  • Supervise and coordinate care with home health aides and other team members.
  • Maintain accurate, timely documentation in compliance with hospice standards.
  • Participate in IDT meetings and promote a culture of empathy and excellence.
Qualifications
  • Current Registered Nurse (RN) License in Pennsylvania.
  • Clinical nursing experience (hospice or home health preferred).
  • Strong communication, organizational, and interpersonal skills.
  • Valid driver’s license, reliable vehicle, and current auto insurance.
  • Ability to travel within the assigned service area.
