Admissions RN-Days-Hospice-Penn Medicine

Job Summary

The Admission RN is responsible for the entry of patients and families into Penn Medicine Hospice. As a member of the Interdisciplinary Team (IDT), the Admission RN provides direct care to adult patients and demonstrates safe practice across clinical judgment, nurse-patient/family relationships, clinical leadership, and scholarship. The RN works with the health‑care team to coordinate care across the continuum, reviews potential client documentation, arranges initial contact, and communicates plans to patients, families, and referral sources.

The RN provides assessment to assure appropriate care levels, facilitates symptom management, and practices within hospice regulations and reimbursement criteria.

• Continuous Quality Improvement (CQI): utilize data and QI methods to identify problems and opportunities for safe, timely, efficient, effective, and equitable care.
• Evidence‑Based Practice and Research: evaluate and integrate the best current evidence with clinical expertise and patient/family preferences for optimal health care and system effectiveness.
• Leadership: collaborate and apply innovative, systems‑thinking approaches to engage in evidence‑based problem solving and decision‑making that promote effective change within a complex care delivery system.

• BLS/CPR, as a healthcare provider per the American Heart Association
• Car Insurance (Required)
• Driver’s License (Required)
• Registered Nurse – PA (Required)

• Bachelor of Science (Required)
• BSN from an accredited nursing school
• More than 15 months relevant professional nursing experience
