RN - Hospice

Job Summary:

We are seeking a Hospice RN Case Manager to join our team at Virtua Health, located at 301 Lippincott Drive. The position is full‑time, 40 hours per week, first shift, on‑site.

• Provide compassionate, home‑based hospice care to terminally ill patients in New Jersey.
• Perform nursing process assessments, diagnoses, planning, implementation, and evaluation at patients’ homes.
• Monitor vital signs, administer medications, and assist with wound care and comfort procedures.
• Educate families on symptom management, disease progression, and prognosis.
• Collaborate with physicians, interdisciplinary team, and referral sources to coordinate care.
• Order medications, supplies, and durable medical equipment as needed.
• Develop individualized care plans and review progress with families.
• Document nursing activities and observations promptly and accurately.
• Participate in bi‑weekly interdisciplinary team meetings and QAPI activities.
• Provide crisis intervention, pain assessment, and appropriate pain management interventions.
• Assist with death pronouncement, documentation of body care, and coordination of funeral or EMT services.
• Perform on‑call, holiday, and weekend coverage as required.
• Travel statewide to meet patients in their homes.

• Minimum of 2–3 years of hospice nursing experience; home‑care experience preferred.
• Graduated from an accredited nursing program; BSN preferred.
• Current New Jersey registered nurse license.
• Current CPR and BLS certifications.
• Valid driver’s license and auto insurance.
• Excellent assessment skills and clinical judgement.
• Knowledge of the physical, psychosocial, and spiritual needs of terminally ill patients and their caregivers.

Hourly Rate: $44.61 – $61.97, varying with experience and market alignment.
