Job Details

Home clinic will be Middletown, but may need to travel to all four locations as needed – Enola, Camp Hill, Middletown and Newberry. Position hours are 8:00am to 4:30pm, Monday through Friday, with light on‑calls and major holidays off.

No prior RN dialysis experience is required. The role includes 12weeks of paid & structured training with an assigned preceptor & nurse educator. Up to $4,400 tuition reimbursement per calendar year.

About this role

As a Home Therapies Registered Nurse you will manage patient care in an independent home therapy program by providing education and support to patients and caretakers who have chosen to dialyze in the comfort of their home. This role is primarily clinic‑based; you will conduct an initial visit to patients’ homes to assess home therapy readiness, then provide regular training and follow‑up visits within the clinic environment.

All new hires join our Clinical Advancement Program (CAP). CAP provides opportunities to develop clinical leadership skills, share expertise with others, and be recognized for experience, knowledge, and clinical expertise. Your CAP level will be based on prior experience and education.

Principal duties and responsibilities
  • Participate in decision‑making, teaching, leadership, and quality improvement activities that enhance patient care outcomes and home therapies program operations.
  • Identify and evaluate candidates for the Home Therapies Program, including assessment of patients’ home environment if needed.
  • Perform assessment and identify barriers of the Home Dialysis candidate’s home environment and caretaker readiness.
  • Admit new patients to the Home Therapies Program using the appropriate processes.
  • Coordinate patient care including assessment, planning, intervention, and evaluation for an assigned group of hemodialysis patients.
  • Assess and manage patients’ response to home dialysis training and treatment therapy by following prescribed protocols.
  • Analyze dialysis data for Home Therapy patients and adjust treatment plans as needed.
  • Provide initial and ongoing education to patients and families on home dialysis treatment and the operation and maintenance of all home therapies equipment.
  • Ensure patient awareness related to transplant and treatment modality options.
  • Administer medications as prescribed or in accordance with approved algorithm(s) and document justification and effectiveness.
  • Initiate or assist with emergency response measures.
  • Complete routine patient evaluations within the clinic setting.
  • Ensure correct laboratory collection, processing, and shipping procedures.
  • Collaborate with the interdisciplinary team on the patient care plan.
  • Make referrals to Social Worker and Registered Dietitian as appropriate.
  • Complete CAP requirements to maintain or advance within the program.
Education

Graduate of an accredited School of Nursing. Current appropriate state licensure. Current or successful completion of CPR BLS Certification. Must meet the practice requirements in the state of employment.

Experience and required skills
  • Qualities and traits: compassion, caring, support for colleagues, collaborative, reliable.
  • Experience as a Registered Nurse (preferred but not required).
  • Chronic/home hemodialysis experience (preferred but not required).
  • Successfully pass the Ishihara Color Blind Test.
Physical demands and working conditions

The role involves direct patient care that regularly involves heavy lifting, moving of patients, and assisting with ambulation. Frequent, prolonged periods of standing, bending over, and moving machines and equipment of up to 200lbs. may be required. The work environment is a health care facility with air‑temperature control and moderate noise levels. Exposure to infectious and contagious diseases/materials may occur. Day‑to‑day work includes desk work, computer work, interaction with patients, facility staff, and physicians.

Travel to patient homes, training sites, or other facilities may be required, and essential functions may be performed at other locations with the same physical demands.
