The Licensed Practical Nurse (LPN/LVN) is responsible for providing and documenting skilled nursing care under the supervision of a Registered Nurse, following the developed care plan and physician orders while complying with confidentiality standards and professional boundaries.

• Delivery and coordination of quality patient care in compliance with physician orders.
• Continuous observation and assessment of patient condition, reporting changes to supervisor/physician.
• Timely documentation of all activities, assessments, nursing actions, responses, and care coordination.
• Participation in, implementation of, and updating the nursing care plan.
• Executing nursing actions based on assessment to achieve expected outcomes.
• Recognizing changes in patient needs and implementing interventions to prevent or reduce risk.
• Compliance with Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act.
• Providing infection control measures in accordance with OSHA standards.
• Educating patients and families on disease process, self‑care, and prevention strategies.
• Maintaining competency through educational programs, continuing education units, and skill evaluations.

• Graduate of an accredited nursing school.
• Current, unrestricted state license as an LPN/LVN.
• Current CPR certification (hands‑on).
• Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care policies.
• Reliable transportation.
• State‑specific requirements:
  • South Carolina – 1 year of pediatrics experience.
  • California – 1 year of experience under current nursing license.
  • Louisiana – 1 year of experience as a licensed nurse.
• Additional preferred experience of 6 months recent nursing in a clinical setting or home health experience.

• Respond physically and mentally to emergencies in the home or during transport.
